Step 1) Obtain emergency assistance as required (e.g. police, ambulance, fire department).

Step 2) Prevent deterioration of the situation. Provide assistance for any injured parties.

Step 3) Fill out an incident report form as soon as possible, while details are still fresh and those involved are still at the scene. Ensure that the form is legible and complete.

Step 4) Obtain witness statements including their name(s), address(es), and telephone number(s). Please ensure comments are restricted to factual statements only - avoid narratives and opinions.

Step 5) If possible, take photographs, videotape or draw a diagram of the incident or accident site (include warning signs, posters, etc.).

Step 6) Call/Email Gougeon Insurance Brokers as soon as possible after the incident occurs.
